A leading company in behavioral health services is seeking a Case Manager to support individuals with mental illness. The role involves case management and direct support to help clients live independently. Candidates must have relevant experience and a commitment to community service.
What You Will Do to Change Lives
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full Time, 40 hours per week, Monday - Friday, 8:00am - 4:30pm
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

CARE Court Assertive Community Treatment (ACT) is opening in partnership with San Diego County Behavioral Health in response to California’s Community, Assistance, Recovery, and Empowerment (CARE) Act.
CARE Court ACT will deliver time-limited, community-based, holistic support to people who meet CARE criteria and agree to participate in these services. Our person-centered approach to care meets people wherever they are in their recovery — addressing the complex needs of the individual and connecting them to resources to thrive in their community.

Program Type: Community-based
To Make Referrals: A referral to CARE Court can be initiated by family members, behavioral health providers, first responders, or other approved petitioners by filing a petition with the Superior Court. Referrals to Telecare CARE ACT are made by the County of San Diego.
