The Role
As a Case Manager at Lifeworks Employment Services (LES), you'll be responsible for facilitating community and program-based activities to encourage self-advocacy and skill building. Your leadership will enhance individuals’ volunteer and employment experience. You will provide job coaching, guidance, and physical assistance when necessary to achieve a person's individual and employment goals.
Through direct support, you will be motivating the people we serve to be empowered in their programs and communities and will have the ability to make a real difference in the lives of people with intellectual and developmental disabilities.

What We’re Looking For
We're searching for a detailed and enthusiastic individual who will bring their personality, an independent work ethic, a strong belief in enhancing the lives of those we serve and strong teamwork skills to join us as a Case Manager in our Employment Training and Support Programs. Are you someone who pays attention to the details, stays organized in a sometimes fast-paced environment, and can exercise good judgment in challenging situations? What You'll Do
- Collaborating with an exceptional team to plan and engage in the advocacy and support of our individuals, attending regular meetings as necessary.
- Supporting individual’s self-employment, education, social growth, health wellness, communication skills, community mobility, and volunteerism.
- Provide support through case management (electronic (Icentrix) and physical case records).
- Writing and implementing Individual Support Plans (ISP) and completing all documentation.
- Monthly and weekly progress notes for individual goals and objectives. Maintaining consistent communication with individuals and members of the ISP team.
- Safely operate company van within the legal guidelines of Massachusetts. Assisting individuals in meeting the expectations of employers at GSWS.
- Provide on and off-site support for Life Enhancement Curriculum: o Assisting individuals served in areas of employment, social, creative, and health & wellness development and training.
- Leading individuals served in community volunteer and learning experiences. Daily completion of payroll and billing documents.
- Other duties and/or assignments at our different locations based on the needs of the team and the individuals you serve.

Education And Experience
- A Bachelor's degree or an Associate’s degree in a related field with three years of experience, or Five years of related work experience in Human Services.
- Effective oral and written communication skills
- Car/Valid driver’s license in State of residency.
All CASE MANAGER candidates must be able to successfully pass a CORI, reference, multi-state background, and driving record check.
