Job summary

Baptist Health’s Emergency Department seeks a Case Manager - RN (BSN required) to coordinate patient care across disciplines and improve outcomes. The role is housed in a 19-bed ED with fast-track capabilities and a focus on safe, cost-effective care.

Qualified candidates will hold an RN license, a BSN, and be prepared to obtain a Case Management certification within two years. Full-time shifts vary, with benefits including comprehensive health coverage, tuition reimbursement, and PTO.

Qualifications

• Current RN licensure recognized by the state board is required.
• BSN is required.
• Must obtain Case Management certification (ANCC/ CCM or ACM-RN) within 2 years if not already held.

Responsibilities

• Assess plans and facilitate options and services meeting patients' healthcare needs in continuum of care.
• Formulate goals with patients and families.
• Advocate for patients through communication and resource evaluation to promote quality outcomes.
• Implement standards of care for case management including identification, planning, monitoring, and transitions of care.
• Perform nursing care activities with supervision and ensure safety and patient satisfaction.

Job Title: Case Manager - RN (BSN REQUIRED)

THIS POSTION IS HOUSED WITHIN OUR EMERGENCY DEPARTMENT

FULL TIME - SHIFTS MAY VARY

The Emergency Department is a 19 bed/ 4 chaired area with a separate 2 bay, 1 bed clinical decision area designated for patients ranging in age from birth to death with health care needs from nonemergent to critical. The unit has designated rooms used for life-threatening conditions with special equipment and supplies, and rooms designated as fast track for non-emergent patients

Job Description

The Case Manager strives for excellence in direct patient care by building a collaborative caring work environment coordinating care and information across the various disciplines focused on patient outcomes and wellness.

• Assesses plans and facilitates options and services which best meet the patients' healthcare needs in the continuum of care.
• Formulates goals with the patient and their families.
• Advocates for the patient through communication and evaluation of available resources to promote quality cost-effective and safe outcomes.
• Incorporates standards of care for Case Management including identification/selection of patients for case management services, problem identification, planning, monitoring, evaluating, transition of care and outcomes.
• Implements practice based on current research and evidence-based practice and ensures adherence to hospital and department policies, regulatory standards, and guidelines.
• Supports an environment of caring focused on the patient's healing and wellness.
• Ensuring Safety, Compliance and Patient Satisfaction.
• Delegation of nursing care activities and provisions of supervision; establishes and maintains a plan for the delivery of nursing care; assures that nursing care interventions and activities are directed to resolve problems; and performs technical aspects of care competently.
• Has access to medication storage areas as required for job tasks. Follows Hospital and Departmental Policies and Procedures.

Requirements

• Current RN licensure recognized by the Kentucky State Board of Nursing is required.
• Bachelor of Science in Nursing Required
• 5 years' experience in acute nursing, case management, utilization management or related field.
• Completion and maintenance of BLS.
• Successful completion of all competencies as identified on the departmental nurse competency grid.
• Must obtain Case Management certification (ANCC, CCM or ACM-RN) or other approved Professional Certificationpreferred upon hire, required within 2 years
