Job summary

BJC HealthCare's Christian Hospital in St. Louis, MO is seeking a compassionate RN Case Manager to coordinate patient care, facilitate safe discharges, and connect patients with post-acute resources.

This full-time, day-shift role requires RN licensure, a nursing degree, 2–5 years of experience in case management or the ED, and EPIC experience. BSN preferred; comprehensive benefits and retirement plan are offered.

Christian Hospital is seeking a compassionate, organized, and collaborative RN Case Manager to join our team. The ED Case Manager plays a vital role in coordinating patient care, facilitating safe and timely discharges, reducing avoidable admissions, and connecting patients with appropriate post-acute resources.

The Case Management Department provides services through a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's needs. The Case Management Department is responsible for all acute care inpatients where needs have been identified or who have requested assistance, emergency department patients where needs have been identified, and community individuals who request assistance.

Preferred Qualifications

Role Purpose

Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues. Performs and provides consultation regarding Utilization Management and Performance Improvement programs in order to obtain optimum value for the patient and the reimbursement source.
