Cape Cod Hospital in Hyannis, MA is seeking a Case Manager to coordinate patient care, plan discharges and work with an interdisciplinary team to promote high quality, cost-effective care.

You will assess patient needs using InterQual criteria and Care Maps, advocate for patients with families, payers and the healthcare team, and facilitate timely transfers. Schedule: 40 hours/week, rotating day shifts, every other weekend and holiday.

Qualifications

• Must be registered as an RN in MA.
• Bachelor of Science in Nursing preferred.
• Case management certification or CPUM preferred.
• 3 years acute care experience within the past 5 years.
• Knowledge of InterQual or equivalent system.

Responsabilités

• Screen new admissions daily for discharge planning needs.
• Coordinate care with interdisciplinary team.
• Assess patient needs per Care Maps and guidelines.
• Facilitate third party reimbursement through reviews.
• Advocate for patients with families, payers and clinicians.
• Participate in daily rounds and care conferences.
