Seven Counties Services is looking for a full-time case manager in Louisville, Kentucky. The role involves conducting comprehensive assessments of clients' needs, developing service plans, and providing advocacy on behalf of clients. Candidates should possess a Bachelor’s degree in a relevant field and demonstrate strong interpersonal and communication skills.

Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.
Minimum of zero years’ experience in case management/service coordination with a Bachelor’s degree.
Working knowledge of mental and physical characteristics of assigned population group (e.g., growth and development), human behavior, treatment/service options, and related issues. Working knowledge of SCS programs and services, policy, and procedure. Working knowledge of applicable regulations and licensure standards. Good interpersonal skills and written and oral communication skills.
Position requires lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing. Normal tasks may involve exposure to blood, bodily fluids, or tissue. May involve 24-hour on-call responsibilities. Reliable transportation for frequent travel and a current driver’s license are required.
