Case Manager - Dakota County ($1,500 sign-on bonus)

Description

Our Dakota County-based Case Managers enhance the quality of case management services, maximize satisfaction, and promote cost effectiveness. The successful candidate will be responsible for the full individual's care cycle, including assessing needs, evaluating treatment options, creating care plans, coordinating care, and monitoring progress to meet health and human services needs.

Are you looking for an opportunity to make a difference in the lives of people with disabilities? At ACCORD, we are seeking Case Managers to work with residents of Dakota County. Our role involves improving service quality, ensuring client satisfaction, and managing the full care cycle, from assessment to monitoring.

Compensation

  • $45,000-52,000 (dependent on experience)
  • $1,500 New Hire Bonus

Job Location

Based in St. Paul, MN, with community-based work with individuals supported.

Essential Responsibilities

  1. Develop community support plans (goals) for each individual (caseload typically 40-45 individuals).
  2. Assist individuals in accessing services, developing service agreements, and documenting through case notes.
  3. Inform clients or legal guardians about suitable service options.
  4. Meet with each individual at least twice a year to evaluate progress and adjust plans as needed.
  5. Identify potential providers to guide individuals toward their goals.
  6. Evaluate and monitor services in the plan.
  7. Follow case management regulations (Rule 185).
  8. Comply with MN Health Care Programs related to waiver services (CADI, BI, EW, DD), including MnChoices assessments and state system usage.

Requirements

  • Meet ONE of the following criteria:
  • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or related field. If in Social Work, current licensure as a Social Worker per Minnesota Board of Social Work is expected.
  • OR Bachelor's in any field with at least one year of experience working with individuals with mental illness or disability.
  • Strong written and oral communication skills.
  • Ability to work independently and in crisis situations.
  • Comfortable with high-volume paperwork.
  • Attention to detail and process-oriented.
  • Ability to pass DHS background check, possess a valid driver’s license, and maintain a clean driving record.
  • Reliable transportation.

Work Environment

Home base in our St. Paul office, with community and office-based work. Typically Monday–Friday, daytime hours, full-time, non-exempt, 40 hours/week.
