Job summary

The GEO Group, Inc. is hiring a Case Manager in San Francisco, CA. The position provides structure to residents as they develop program plans, manages weekly progress reviews, and assists with budgeting, housing, education, and community reintegration.

Responsibilities

  • The Case Manager provides support and structure to residents in developing program plans.
  • Reviews plan with residents on a weekly basis and documents progress and/or revisions.
  • Assists residents in establishing personal budget, housing, education and vocational programs to pursue financial independence.
  • Monitors facility sanitation and safety; assigns daily housekeeping chores to residents to comply with facility standards.
  • May orient participants upon arrival and provide smooth assimilation into the program.
  • Maintains confidential participant files to support program analysis and documentation.
  • Verifies pass plan through pass sponsorship according to contract and ensures accountability in the community.
  • Prepares terminal report within established time parameters upon resident release.
  • Participates in staff meetings to discuss facility, residents, and staff relations.
  • Maintains confidentiality of information with regard to employees, vendors, residents, and outside agencies.
  • Adheres to attendance policy and is available for all shifts, including overtime as required.
  • Complies with safety rules and promotes safe work practices.
  • Communicates effectively with all levels of staff to foster teamwork.
  • Performs other duties as assigned.

Responsibilities
Summary

The Case Manager provides support and structure to residents in developing program plans. Controls and secures facility and residents and operate within legal constraints and contract agency stipulations.

Primary Duties And Responsibilities
  • The Case Manager prepares written personalized programs for each participant to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals.
  • Reviews plan with residents on a weekly basis and documents progress and/or revisions, so that residents are gaining the life skills necessary to make a successful re-entry into the community.
  • The Case Manager Assists residents in establishing personal budget, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities, as appropriate, to ensure that they are working towards financial independence.
  • Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards.
  • May orient participants upon arrival at facility to provide smooth assimilation into program including having them complete paperwork, take facility tours, or introduce to other participants.
  • The Case Manager makes entries into confidential participant files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensure files are secure to protect resident’s confidentiality.
  • Verifies participants' pass plan through pass sponsorship according to contract so that the participant is accountable for his/her whereabouts in the community.
  • The Case Manager prepares terminal report within established time parameters upon resident release to summarize program performance and to satisfy contract requirements.
  • Participates in staff meetings by sharing observations and recommendations regarding the facility, residents, and staff relations to identify and solve problems and function as a unified team.
  • Maintains confidential information obtained through job duties regarding employees, vendors, residents, outside agencies, etc., so sensitive information is on a "need to know" basis.
  • Complies with GEO Reentry policies and work rules as described in the company Employee Handbook, and the Facility Policies and Procedures Manual, including directives of GEO Reentry managers and supervisors in a cooperative spirit to ensure conformance to company standards.
  • Adheres to GEO Reentry's attendance policy; be available for all shifts; work overtime as required; to ensure adequate coverage of the program and to ensure the safety and security of the residents.
  • Complies with facility safety rules; take appropriate corrective action to ensure work is performed in a safe manner and without injury to self or others.
  • Communicates effectively with all levels of staff in a manner that promotes a team spirit and a sense of cooperation.
  • Performs other duties as assigned.
Qualifications
Minimum Requirements
  • Bachelor’s degree in a social, human services, criminal justice or behavioral science program from an accredited college or university.
  • One (1) year of experience must be working in human services, corrections, or community-based services.
  • Work experience may be substituted for academic studies exchanging one (1) year of work experience in a related field for one (1) year of academic education. Total work experience needed in lieu of the combination of education and work experience is five (5) years.
  • Familiar with community corrections systems and current community corrections practices.
  • Familiar with community work force needs, public and private agencies, and employment services.
  • Ability to write clear concise reports in English using correct grammar.
  • Ability to add, subtract, multiply, divide, and calculate percentages.
  • Must be able to react quickly and appropriately in an emergency situation.
  • Bilingual in English/Spanish preferred, but not mandatory.
  • Ability to work with computers and the necessary software typically used by the department.
