Lutheran Social Services of Illinois (LSSI) is hiring a full-time CST Case Manager in Chicago, IL. This role delivers whole-person services within mental health case management programs, including 1:1 meetings, case management assistance, and evidence-informed interventions to support behavioral, emotional, and social skill development.

The position collaborates with treatment professionals and community partners to help clients achieve goals.

Qualifications

• High school diploma or equivalent required.
• Associate’s, bachelor’s, or master’s degree in a social service field preferred.
• If master’s level, work toward clinical licensure.
• LPC/LCPC or LSW/LCSW preferred.

Responsibilities

• Meet and exceed revenue productivity.
• Carry a client caseload to meet 60% productivity (24 billable hours per week).
• Collaborate with clients and colleagues to complete lM+CANS, treatment plans, and other required documentation.
• Understand and follow all compliance rules regarding treatment of services and contracts.
• Reflect Equity, Diversity, and Inclusion in clinical practice.
• Adhere to timelines for documentation completion.
• Ensure adherence to NASW Code of Ethics.
• Demonstrate professional, respectful conduct with clients and colleagues.

LSSI is hiring a full-time CST Case Manager, Monday through Friday. Responsible for providing whole-person services to clients participating in mental health case management programs, including 1:1 meetings, case management assistance, and the utilization of evidence informed interventions that target behavioral, emotional, and social skill building. Also responsible for assisting clients in achieving their stated goals and objectives; collaborating with internal and external treatment professionals and community partners.

Essential Functions

• Meet and exceed revenue productivity.

• Understand and reflect the clinical model of the program, demonstrating principles of Equity, Diversity, and Inclusion.
• Adhere to all timelines regarding documentation completion.
• Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
• Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individuals in a dignified, respectful, honest, and fair manner.

Position Qualifications

• High school diploma or equivalent required.
• Associate’s, bachelor’s, or master’s degree in a social service field preferred.
• If master’s level, work toward clinical licensure.
• LPC/LCPC or LSW/LCSW preferred.
• Background check clearance required.
• Demonstrated proficiency in Electronic Health Record documentation and review required.
• Clinically trained and experience using best practices as well as experience working with a variety of client ages, issues, and symptoms preferred.
• Familiarity with accreditation and licensure standards preferred.Experience establishing and maintaining collaborative community partnerships preferred.
• Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
• Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
• Must Have:
• Valid driver’s license, in good standing for the state of residency required.
• Access to reliable transportation required.
• Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
