Job summary

North Florida Rehabilitation Hospital, part of Lifepoint Health, seeks a Licensed Case Manager to coordinate interdisciplinary care plans and discharge planning in an inpatient rehab setting. You will liaise with patients, families, and the care team to ensure smooth transitions and timely updates.

The role emphasizes strong communication, case management in a rehab context, and collaboration with medical and non-medical staff.

Qualifications

• Licensure required as RN, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist or Speech-Language Pathologist.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of inpatient social work or case management experience preferred; acute rehab hospital experience strongly preferred.
• Proficiency in English communication; MS Office skills required.

How you'll contribute

The Licensed Case Manager is responsible for coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting. This role serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.

What we're looking for...
