Position Summary
The Case Manager is responsible for coordinating healthcare decisions using a systematic approach to ensure treatment plans that improve quality and outcomes. This includes care coordination across the continuum, promoting cost-effective care within designated timeframes, ensuring hospital-based services meet patient utilization management criteria, and implementing safe discharge plans. The Case Manager assesses social work needs and provides interventions as part of discharge planning.

Each patient receives personalized treatment plans based on their medical and therapy needs.
Select Medical emphasizes safety and conducts post-offer employment testing (POET) for various roles, including nursing, therapy, and environmental services, to be completed before employment begins.
