Position Summary

The Case Manager is responsible for coordinating healthcare decisions using a systematic approach to ensure treatment plans that improve quality and outcomes. This includes care coordination across the continuum, promoting cost-effective care within designated timeframes, ensuring hospital-based services meet patient utilization management criteria, and implementing safe discharge plans. The Case Manager assesses social work needs and provides interventions as part of discharge planning.

Responsibilities

  • Clinical Interventions/Discharge and Care Planning Management: Acts as liaison among all parties involved with the patient within the hospital and community to develop and implement safe and appropriate discharge plans. Participates in developing, implementing, reviewing, and revising the interdisciplinary care plan.
  • Fiscal Management: Maintains responsiveness to payer systems by communicating with external payers, generating revenue, enhancing reimbursement, minimizing financial risks, and ensuring payment for hospital services. Interprets insurance coverage and benefits for patients and advocates as needed.
  • Payer/Referral Management: Fulfills payer and referral needs, oversees negotiations regarding stay rationale, duration, and appeals, and develops positive relationships with payers and referral sources to promote hospital reputation and business.
  • Coordinates with departments such as Pre-Admissions, Admissions, Patient Accounts, Utilization Review, PPS, etc., to achieve positive fiscal outcomes.
  • Develops and maintains effective interdisciplinary relationships with medical, nursing, therapy, psychological, dietary, and vocational team members to ensure appropriate, cost-effective clinical outcomes.

Minimum Qualifications

  • Current and valid state licensure in Social Work or Nursing (RN).
  • Masters Degree in Social Work or BSN in Nursing preferred.
  • Experience in case management or related field required.
  • CCM Certification preferred.
