Position Summary: The Case Manager is responsible for providing direct and indirect casework services to patients and their families, as well as rehabilitation and skill development services.
Essential Duties and Responsibilities include the following and other duties as assigned.
- Provides behavioral health case management services by linking, resourcing, advocating, and providing.
- Assists the team in identifying community and agency resources to meet patients’ needs, such as recovery programs, local sponsors, outreach programs for continued recovery.
- Links Patients to Sooner Care online portals, updates information, assists with completion of online applications for employment, unemployment benefits, etc.
- Advocates on behalf of Patients regarding unemployment, benefits, and services (requires ROI).
- Provides resource information for Patients: e.g., local food distribution times and locations, clothing closets, soup kitchen times and dates, etc.
- Provides information and addresses, e.g., potential jobs, recovery groups, faith-based family-friendly events (e.g., Easter hunt, fireworks displays, holiday events and dinners).
- Ensures necessary forms are provided to the Medical Director, properly and accurately completed, especially when patients have special requests for take-out medications, dose changes, phase changes, MSW schedule, and medication changes.
- Ensures patient confidentiality by properly completing release of information forms for all outside referrals and contacts (e.g., medical coordination, information from other facilities).
- Completes discharge procedures within fourteen (14) days of patient discharge, with the discharge form fully completed and the file submitted to the Program Director.
- Contributes to ongoing assessment and treatment planning through participation in daily reports, case reviews, and treatment teams.
- Participates in professional development, including external and internal in-service programs and events.
- Works with medical staff to secure urine samples for monthly urinalysis, documents results in the patient’s chart and in Methasoft.
- Involves actively in Diversion Control through execution of random bottle recalls.
- Manages and prioritizes caseload, providing behavioral health case management to patients and families.
- Acts as advocate and intermediary for patients, ensuring provider needs are met.
- Provides decision-making information to individuals/guardians/family members/advocates with appropriate releases.
- Resolves problems and mediates with providers, agencies, and team members.
- Performs other duties as assigned by the Program Administrator.
Job Standards/Minimum Qualifications:
Certified Behavioral Health Case Manager I must meet one of the following:
- Have 60 college credit hours; or
- Possess a high school diploma or equivalent from a regionally accredited institution recognized by the U.S. Department of Education, with at least 36 months of documented experience working with individuals with mental illness and/or substance abuse issues.
Must have completed ODMHSAS required training and passed CMI competency exams.
Physical Requirements:
- Frequent sitting at desk or computer, typing, completing paperwork, or meeting with patients.
- Occasional lifting/carrying of up to 20 lbs., paperwork, files, or office supplies.
- Occasional push/pull exerting up to 5-10 lbs. force, opening doors, file drawers, pushing files in drawers.
- Occasional kneeling/crouching and reaching lower file drawers or stretching to store or retrieve materials.
Mental Requirements:
- Constant mental alertness, attention to detail, and high accuracy in completing assignments.
- Ability to follow oral and written instructions and ensure follow-through on all tasks.
