Job summary

Virginia Mason Franciscan Health Rehabilitation Hospital, operated jointly with Lifepoint Health, seeks a dedicated Case Manager to deliver exceptional patient care. You will complete IPoC per CMS guidelines, collaborate with an interdisciplinary team, manage documentation within workflow timelines, and lead family conferences and care-team reviews.

Applicants should hold a bachelor’s degree in nursing or social work, with BLS due within 30 days and ACLS/PALS within six months.

Qualifications

  • Bachelor's degree from an accredited nursing school or Social work program.
  • Basic Life Support (BLS) certification required within 30 days of hire.
  • ACLS and PALS required within six months of hire.
  • One year of experience preferred.
  • ASLS certification preferred.

How you'll contribute

A Case Manager who excels in this role:

  • Complete departmental orientation as well as initial and annual competencies.
  • Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
  • Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
  • Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
  • Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
  • Perform other duties as assigned.

What we're looking for

Applicants should possess a bachelor's degree from an accredited nursing school or Social work program. Additional requirements include:

Salary Range: $93,600--$104,000 Per year
