Description

This position will cover areas in North County San Diego including Poway, Mira Mesa, Del Mar, Oceanside, Vista, Escondido, and Ramona. Position Purpose

The Case Manager provides administrative and programmatic support to the Area Manager for the operations of programs, client management, and support of program staff. The Case Manager provides job-site-based support, coaching, work direction, and instruction to clients with intellectual and/or developmental disabilities (IDD) within the program.

Essential Functions and Responsibilities

  • Serve as a coach to program staff and provide work direction to ensure completion of daily report entries with adequate detail of client engagement.
  • Act as liaison between DSPs and Area Manager to support the planning, coordination, and delivery of program service, initiatives, program design, and activities for clients in-person and remotely.
  • Assist the Area Manager with the supervision of the planning, coordination, and delivery of program service, initiatives, program design, and activities for clients in-person and remotely.
  • Assist the Area Manager with budgetary and billing support functions.
  • Coach DSPs in how to create and deliver functional curriculum for client activities as requested.
  • Provide feedback to the Area Manager to support staff performance management.
  • Ensure program compliance with regulations, policies, guidelines, and procedures (Quality Assurance).
  • Complete and/or ensure staff complete daily reporting requirements and conduct frequent audits to ensure quality and reporting compliance.
  • Hold staff accountable to deadlines for administrative responsibilities.
  • Assist with the management of daily programming schedules of staff and clients to ensure compliance, quality, and efficiency of service delivery.
  • Manage the system of collecting organizational (internal & external) metrics consistently and accurately in database management system (intake paperwork, renewals, ISPs, semi-annuals, SIR/APS/Program reports and behavior support plans).
  • Manage the system of reporting data collected to all interested parties.
  • Monitor the delivery of daily program support, supervision, and instruction to clients.
  • On occasion serve as backup to provide direct support to clients in the field and/or remotely.
  • Provide training and safety program coordination for staff liaison between DSPs and the region's safety coordinator to assist with training and safety for staff.
  • Participate in the training and/or support new staff in various required areas of program operations (database management and payroll systems, safety documentation, policies and procedures, on-the-job training and support).
  • Additional duties as assigned.

Qualifications

Education

  • High school diploma or equivalent.
  • Bachelor’s degree (preferred).
  • 90+ college units (preferred).

Experience

  • Minimum 4 years of direct, hands-on experience supporting individuals with a wide range of intellectual and/or physical disabilities and medical needs (may have less experience with college units).
  • Experience in community-based instruction in critical life needs areas (preferred).
  • Experience in assisting individuals in the use of adaptive devices and communication aids (preferred).
  • Supervisory and management experience (preferred).
  • Must have and maintain valid drivers’ license, valid auto insurance, acceptable driving record, and ability to travel within and throughout the region where services are provided.
  • Must complete and pass background check.

Physical Demands & Work Environment

This position requires the employee to regularly stand, walk, sit, talk, and hear. They must be capable of writing, using a computer keyboard, telephone, and related office equipment. Ability to lift 15lbs.
