Case Manager - Care Management for Adults

Catholic Charities of the Archdiocese of Newark is one of New Jersey’s largest non-profit social service agencies with programs and facilities serving Bergen, Essex, Hudson, and Union counties.

We are currently seeking a Full Time Case Manager for our Care Management for Adults & JACC program in Hackensack, NJ.

Job Duties:

• The Case Manager is responsible for developing and managing service plans for clients, including direct counseling services.
• Formulate and implement individualized service plans.
• Establish continuity of care and ensure delivery of appropriate quality services.
