Responsibilities
  1. Coordinate and facilitate interdisciplinary, patient-centered, quality healthcare throughout the continuum for patients with acute and chronic conditions.
  2. Foster achievement of optimal health outcomes within accepted standards of care.
  3. Serve as an expert resource regarding the continuum of care, resource utilization, best practices, team-based care, quality indicators, and regulatory requirements.
  4. Ensure smooth transitions of care between healthcare environments with planned handoffs.
  5. Partner with patients and families to identify healthcare issues and barriers, set priorities, and implement interventions.
  6. Provide education on self-management strategies, employing cultural agility and health literacy guidelines.
  7. Utilize quality improvement cycles to monitor and evaluate interventions and outcomes.
  8. Manage care pace to ensure appropriate and fiscally sound coordination across the continuum.
Qualifications
  • Education: BSN or higher preferred.
  • Experience: Minimum 2 years clinical nursing experience, with knowledge of disease management, healthcare reimbursement, and community resources.
  • Preferred experience in community resource coordination, care management of diverse populations, or ambulatory care.
Knowledge and Skills
  • Understanding of the six core components of case management.
  • Excellent organizational and communication skills.
  • Ability to determine and access community resources.
  • Ability to engage patients and families considering cultural and health literacy factors.
  • Knowledge of regulatory issues and ability to implement regulations effectively.
  • Proficiency in electronic systems navigation.
Additional Information

Salary range: $50.57 - $75.55 hourly.
