Pinnacle Services is an innovative agency experiencing rapid growth in the Case Management department. Do you have a bachelor’s in social work? Are you licensed in Social Work? Case Manager Team Leads have significant responsibilities while managing their caseloads and their team; therefore, someone who seeks opportunities to consult, collaborate, and lead others would excel in this role.

Job Responsibilities and Duties:

Case Manager Captains work with individuals who may have developmental disabilities, serious and persistent mental illnesses, chemical dependency issues, physical disabilities, or are aging.

The Case Manager Captain is responsible for:

  • Assessing the areas of the individual's life where assistance is needed.
  • Offering and coordinating resources for the individuals being supported.
  • Monitoring service delivery and working with providers to ensure the individual's needs are met.
  • Developing and implementing service plans.
  • Attending regular meetings with individuals served and their interdisciplinary teams.
  • Leading a team of 6-9 Case Managers.
  • Answering questions and providing consultations as needed.
  • Training and supervising subordinates.
  • Staying current with systems and software used in Ramsey County.

Salary:

$54,000 per year (salaried)

Requirements

Required Qualifications:

  • At least 1 year of Waivered Case Management experience
  • Previous supervisory experience leading a team of 5 or more
  • Valid driver’s license, acceptable driving record, and vehicle with current insurance
  • Successful clearance of local and/or state background checks

Preferred Qualifications:

  • LSW or higher BOSW licensure
  • One year of experience in education or treatment of individuals with developmental disabilities or related conditions, or coursework focusing on developmental disabilities
