ReMed, a Collage Rehabilitation Partner in Malvern, PA, seeks a Rehabilitation Case Manager to coordinate client treatment plans from admission through discharge. You will act as a primary advocate, bridging clinical teams, families, physicians, and funding sources to ensure cohesive, high-quality care.

You will lead case management activities, monitor progress toward rehabilitation goals, and collaborate across disciplines to meet funding and regulatory requirements while supporting patients

For more than 35 years, we've developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

We are seeking a dedicated Rehabilitation Case Manager to coordinate and oversee comprehensive client treatment plans from admission through discharge. In this role, you will act as a primary client advocate, bridging the gap between clinical teams, families, physicians, and funding sources to ensure cohesive, high-quality care and continuous progress toward rehabilitation goals.

Key Responsibilities

• Case Management & Advocacy: Manage assigned caseloads, lead interdisciplinary treatment team meetings, and advocate for client needs across all phases of care.
• Treatment Planning: Monitor, evaluate, and revise individualized client rehabilitation goals in close collaboration with the Clinical Director and primary healthcare providers.
• Stakeholder Communication: Maintain regular, transparent communication with clients, families, and funders regarding progress, schedule updates, and care expectations.
• Financial & Funder Coordination: Monitor funding guidelines, facilitate alternative or future funding sources, and ensure accurate, timely documentation for billing integrity.
• Clinical Documentation: Maintain accurate client charts, oversee staff log notes, and comply with state regulatory standards and professional guidelines.
• Direct Care & Support: Provide occasional direct care support or transportation to medical appointments as needed; participate in the program's on-call rotation.
