Job summary

Methodist Children’s Home seeks a dedicated Case Manager (Boys Ranch) to deliver social services aligned with the MCH Model of Care. You will support children, youth and families, build rapport, and coordinate with foster families, caregivers and other providers to ensure safe, therapeutic care.

Job description

TheCase Manager (Boys Ranch) reports to the Senior Director for Programs (Ranch)and is responsible for providing social services to children, youth andfamilies that are consistent with the MCH Model of Care and ensuring a safe,caring and therapeutic environment for those served. Thisposition requires the intellectual and emotional maturity to work with children,youth and families who are experiencing difficulty in providing appropriatecare for the children in their care and children who have behavioral issues,developmental delays and mental health needs. This position also requiresrespect and compassionate understanding for these children, youth and families,and the ability to form effective working relationships with children and parents/guardians/managingconservators. CaseManagers must have a thorough understanding of and demonstrated competency inthe Trust-Based Relational Intervention (TBRI) and other evidence-basedprograms offered in the assigned area. Case Manager must maintain a healthyvalue system, including moral and ethical behavior consistent with the agency'smission and core values.

DUTIES AND RESPONSIBILITIES

• Developand implement written plans of service based on the assessed needs of the childor family in accordance with program guidelines. Revise and update the written plans ofservice as needed to address the changing needs of those served and to complywith program guidelines.
• Provideservices in compliance with program guidelines, state standards and allcontracts as outlined in the plan of service.
• Maintaina regular schedule of contacts and case management services on the assignedcaseload in accordance with program guidelines and the needs of each case. Goals and methods related to these contactsare to be established in consultation with the supervisor, parents/managingconservators and, when possible, with the children and youth served.
• Coordinate,consult and collaborate with foster families and/or kinship caregivers andother service providers to provide needed services to children, youth andfamilies.
• Providekinship caregiver home support and/or aftercare support, in compliance withT3C.
• Serve asbehavior support specialist by building rapport with children and caregivers;engaging, supporting, and coaching them in the use of evidence-informedtechniques to improve daily functioning; and fulfilling T3C staffingexpectations.
• Maintainaccurate and timely documentation in the client information system and clientfile, including all relevant contacts, activities, incidents, appointments,service plans, reports, case notes, case logs, correspondence and otherrequired case documentation.
• Keepsupervisor, parents/guardians/managing conservators, and other involved partiesinformed of incidents and other developments in the case as required by programguidelines, agency policies, contract provisions and applicable licensingstandards and accreditation standards.
• Makerecommendations for continuation, closure or change in services.
• Leadparent education groups or other non-clinical groups as assigned.
• Promoteawareness of MCH programs and participate in program recruitment activities.
• Maintaincompliance with agency policies/procedures and licensing, contracts andaccreditation standards.
• Manageagency funds, resources and equipment according to agency policies andprocedures.
• Workcollaboratively in a proactive and positive manner with other departments.
• Representagency at community functions, activities and events.
• Ensureall facilities and equipment are clean, organized and maintained.

WORKING CONDITIONS

This is aprofessional position that may require more hours than the normal 40-hour workweek.Responsibilities include work in the evenings and on weekends. This positionrequires serving in an on-call capacity and responding to emergencies afteroffice hours. Out of town and overnight travel are also required. An automobilefor in town and out of town travel is required. Mileage reimbursement isprovided for use of personal automobile for approved business purposes. A technology stipend isprovided to support the communication requirements of this position.

EDUCATION/ LICENSURE

A master's degree or higher from an accredited college oruniversity in social work or other human services field and nine credit hoursin graduate level courses that focus on family and individual function andinteraction along with at least one year of documented full-time workexperience in a treatment setting serving children is required.

TRAINING

Training hours mustbe completed pre-service and annually to comply with licensing, contract andaccreditation standards, as well as MCH policies and procedures. The employee'ssupervisor may require additional training to meet specific job requirements orenhance professional growth. Completion of training requirements is mandatoryfor continued employment.

OTHER

The Case Manager mustbe at least 21 years of age and have a valid Driver's License, an excellentdriving record that meets the standards set forth by the liability insurancecompany and Methodist Children's Home, no criminal history and acceptablereferences.
