St. Vincent’s Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent’s is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system’s second-largest hospital.
Job Summary:
The Behavioral Health Case Manager provides intensive non-clinical case management services as an integral member of patient care teams in the Emergency Department Behavioral Health Unit. The Case Manager works collaboratively with the patient and their treatment providers to overcome any barriers patient may encounter while accessing our services. The Case Manager provides trauma informed case management to support patients and families in order to assist patients in achieving their treatment goals and objectives during treatment. They help coordinate transitions of care within the department and/or to outside agencies. Provides critical information to members of the patient’s clinical care team to aid in monitoring and adjusting the treatment plan while working closely with the patient, parent, or caregiver. Advocates and represents the voice and needs of the patient when securing services with local, state, and federal agencies, assists the patient in obtaining any basic needs that they or their family may need including but not limited to: outpatient behavioral health treatment, re-connection to current mental health providers, information and access on food stamps, WIC and other food services, transportation out of emergency department, and community assistance by referral to proper resources.
