Humana is seeking a Case Manager, also known as a Community Resource Coordinator (CRC), to join the Inclusa team serving members in the Wisconsin Family Care (FC) program. In this role, you will partner with colleagues to provide top-quality care, assisting members in their daily lives. The CRC offers comprehensive social service care management for frail elders and adults with intellectual, developmental, or physical disabilities eligible for Wisconsin’s FC program.

Responsibilities

• Learn what is important to the member and support them in achieving it.
• Partner with a Health and Wellness Coordinator (RN) and collaborate on the member’s plan.
• Perform face-to-face social assessments upon enrollment and every 6 months.
• Coordinate, perform, and track quarterly face-to-face visits and monthly phone contacts.
• Develop, review, and coordinate the member’s care plan.
• Lead in social, community integration, employment, housing, and non-health issues.
• Educate on prevention, wellness, and risk mitigation.
• Document activities and complete required paperwork.
• Travel within the service area, eligible for mileage reimbursement.

Qualifications

• Bachelor’s degree in Human Services or related field with 1 year of experience with target populations OR Bachelor’s degree in any field with 3 years of relevant experience.
• Preferred: Certification in social work.

Additional Details

• Workstyle: Field position, providing services at various locations including members’ homes.
• Location: Baraboo, WI, and surrounding areas.
• Travel: Up to 40% within the area.
• Hours: Monday – Friday, 8:00 am – 4:30 pm CST.
• Remote work possible within designated geography.
