Horizon Health in Bay Minette, AL is seeking a Part-time Case Manager (Bachelor's in Social Work) for our behavioral health program serving Mobile Infirmary and North Baldwin Infirmary. The Case Manager will assess needs, guide discharge planning, and coordinate community resources under supervision of a Social Worker II.

The role emphasizes collaboration with multidisciplinary teams, family involvement, and timely documentation.

Qualifications

• Bachelor's degree required.
• Experience in case management/discharge planning required.
• 0-1 years preferred.
• Provisional State license in Social Work (LCSW-A) required.
• Valid Driver’s License required.
• Experience in hospital case management preferred.

Responsibilities

• Implements a treatment plan of care for patients under supervision of a LCSW.
• Participates in referral development processes and activities as directed/requested.
• Engages in timely discharge planning with patient/family, nursing staff and attending physician; connects to community resources.
• Participates as an active member of the interdisciplinary and treatment teams; attends Unit and Hospital meetings.
• Conducts individual, group and education psychotherapy sessions as directed; provides group/individual therapies on weekends as directed.
• Delivers therapies aligned with patient diagnosis and needs.
• Provides resources and support for patients before, during, and after treatment.
• Submits weekly CQI+ outcome data.

Case Manager (Bachelors in Social Work) - Part-time - Behavioral Health – Bay Minette, AL. Horizon Health is seeking a Case Manager (Part-time) for our behavioral health program at Mobile Infirmary and North Baldwin Infirmary in Bay Minette, AL. The Case Manager I is responsible for providing high quality patient care, displaying good skills in assessment, guidance and discharge planning. The Case Manager assists patients and their families in understanding and adjusting to diagnoses and potential treatments. Case Manager can also help identify resources during treatment and after discharge. Case Manager effectively participates as a member of the multidisciplinary team and the unit community relations team. Case Manager may be under the direct clinical supervision of a Social Worker II.

Responsibilities:

• Implements a treatment plan of care for patients based on a thorough assessment of the patient’s symptom presentation, professional/referral input and all other available data under the guidance of a LCSW.
• Appropriately participates in referral development processes and activities as directed/requested
• Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources and makes effective contact with disposition sources
• Participates as an active member of the interdisciplinary and treatment teams. Attend Unit and Hospital meetings as directed
• As directed, conducts individual, group and education psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques (under the guidance of a LCSW). Provides group and individual therapies on weekends as directed by supervisor
• Conducts therapies in accordance with patient diagnosis and needs
• Provides appropriate resources and support for patients before, during, and after treatment
• Submits timely and accurate CQI+ outcome data on a weekly basis
• Performs other duties as assigned/required by this position
