Case Manager – Full Time - Auburn
Schedule: Monday-Friday 9:00am – 5:30pm | Rate of pay: $18.00 per hour

We are currently seeking a Full-Time Case Manager to support our work with homeless and housing-vulnerable families and women in Auburn, NY. In this essential role, you will help individuals and families find and maintain permanent housing, connect to vital services, and support them on their journey toward greater independence. Key Responsibilities:
  • Client Case Management: Carry a caseload and maintain accurate, timely documentation for each client’s progress and service needs.
  • Supportive Counseling: Build rapport, encourage, and motivate clients while maintaining professional boundaries.
  • Goal Setting & Planning: Collaborate with clients to identify strengths, set attainable goals, and monitor their progress toward long-term independence.
  • Service Coordination: Refer clients to appropriate internal and external providers, including mental health, addiction, legal, and financial services.
  • Community Engagement: Build relationships with housing authorities, service agencies, and local organizations to expand resource access.
  • Housing Support: Assist with preparing vacant units, basic cleaning, and removing abandoned items.
  • Transportation: Provide transportation for clients to key appointments as needed.
  • Team Collaboration: Participate in regular meetings and coordinate care plans with fellow staff members.
  • Safety & Compliance: Promote a safety-first environment and adhere to all organizational policies and procedures.

Requirements:
  • Bachelor’s degree in Social Work or related field, or equivalent work experience.
  • Master’s degree in Social Work or related field preferred.
  • Previous experience working with vulnerable populations, including those experiencing homelessness.
  • Valid NYS Driver License (must meet company driving criteria).
  • Familiarity with local community services is a plus.
  • Bilingual abilities are a plus.
