Assertive Community Treatment (ACT) is a 24/7 community based, multi-disciplinary team, serving individuals with a severe and persistent mental illness (SPMI) to help them reach their recovery goals in the least restrictive environment possible. The ACT case manager works with the individual and other supports and service providers to develop an Individual Recovery Plan (IRP) that includes the medical, behavioral, social, educational, vocational, housing, financial and other needs of the eligible individual. Services help individuals develop strategies and support to avoid out-of- home placement and reduce the need for more intensive services, and coordinate access to other needed community services and resources.
