• Visits with persons on the caseload and attends team meetings.
• Completes needs assessments and develops individual service plans (CSSP).
• Assists with planning for new service development.
• Locates residential, vocational, and other needed services per each person’s needs.
• Monitors service delivery and ensures health and safety needs are met.
• Completion of referrals and crisis intervention as needed.
• Develops waiver budgets.
• Completes case notes.
• Acts as an advocate for people we provide services to.
