Job summary

Community Solutions seeks a Case Manager for the Watkinson House location in Hartford, CT. The role includes intake assessment, individual and group counseling, case record management, discharge planning, and aftercare services.

Schedule: Mon/Wed 1:00pm–9:00pm, Tue/Fri 9:00am–5:00pm, with a pay rate of $23.84 per hour. Bachelor’s degree in a social science and strong communication skills are required; experience with Microsoft Office is preferred.

  • Prepare, plan, and facilitate Transitional Skills and R&R group sessions.
  • Record all sessions with clients and upload recordings for quality assurance review.
  • Actively participate in quality assurance reviews and apply new strategies and techniques.

Job description

Community Solutions is seeking a Case Manager to join our team at the Watkinson House location in Hartford, CT. The role involves case management duties including intake assessment, individual and group counseling, case record management, discharge preparation and aftercare services.

Schedule

Monday/Wednesday 1:00pm to 9:00pm and Tuesday/Thursday/Friday 9:00am to 5:00pm.

Pay Rate

$23.84 per hour.

Responsibilities
  • Check daily for newly assigned clients.
  • Complete assessments within three business days of intake.
  • Facilitate feedback and create ISP within five business days of intake.
  • Conduct DOC case management sessions based on the client’s assessment scores.
  • DOC clients: submit monthly progress reports to supervisor four calendar days before due date.
  • Home confinement clients: collect and review phone records monthly to ensure compliance with the SOW.

  • Participate in PRT meetings by presenting new client assessments and ISPs, updating on clients nearing discharge, and providing peer support for client issues.
  • Make recommendations to supervisor for corrective action as required and monitor ongoing progress.
  • Maintain client files and ensure compliance with Agency policy regarding confidential information.
  • Assist clients with obtaining birth certificate and social security card.
  • Home Confinement clients (Wilmington, WH, HH, and Chase only):
    • Conduct CM sessions at the client’s home confinement site.
    • Secure urine specimens and Breathalyzer tests at the HC site.
    • Conduct searches of the client’s vehicle at the HC site.
  • On a monthly basis, collect and make copies of client bank records to ensure they balance with our financial records.
  • Complete incident reports as needed and submit to PD prior to leaving shift.
  • Register for and attend all required trainings and boosters, including but not limited to MI, Case Management, and Assessments.
  • Secure urines from same‑sex clients and complete all required paperwork.
  • Monitor case load to ensure all urines and Breathalyzer tests are randomly requested and secured as required per funding source.
  • Conduct weekly audits of all client medications to ensure medication compliance.
  • Conduct employment, job search, and home confinement spot checks.
  • Conduct room, common area, and perimeter searches.
  • Conduct pat/pocket searches, facility rounds, and house counts as needed.
  • Assist with serving meals when assigned.
  • Serve as a role model to clients and staff by modeling pro‑social behaviors.
Qualifications
  • Bachelor’s degree in a social science field and one year of experience in case management or a similar field (education may be substituted with experience on a year‑for‑year basis).
  • Excellent oral, written, and interpersonal skills.
  • Ability to engage and develop collaborations with community agencies.
  • Previous assessment experience preferred.
  • Experience with Microsoft Office.
