Job summary

Beth Israel Lahey Health’s Danvers Treatment Center is seeking a Case Manager to support individuals seeking recovery from addiction. The role emphasizes intake, evaluation, counseling, and coordinating next steps in care within a collaborative, patient-centered program.

Responsibilities include arranging aftercare, referrals to self-help resources, and coordinating transportation. A related degree with at least one year of experience is preferred; a driver's license is recommended.

Qualifications

• Associates or Bachelor’s in Human Services preferred; HS diploma acceptable.
• Minimum one year related experience in addiction treatment.
• Valid Driver’s License preferred.

Responsibilities

• Obtains documents and intake materials.
• Conducts intake evaluation and counseling sessions.
• Participates in client evaluations and case conferences.
• Provides referrals for family members to self-help programs.
• Assesses continuing care needs and arranges aftercare transfers.
• Coordinates transportation to next level of care.
