Case Manager - ADAMH (Alcohol, Drugs & Mental Health)

Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, providing meals and shelter to men, women, and Veterans with dignity and compassion.

What will I do as the ADAMH Case Manager with Lutheran Social Services?
- Work with clients regularly to develop and maintain individualized plans of action, setting goals by assessing client strengths and needs, responsibilities, and action steps. Assist clients with budgeting, job and life skills, maintaining housing, paying bills, applying for benefits, and employment skills.
- Make appropriate referrals and linkages to community resources, supporting clients in securing stable housing and income. Act as a liaison with outside community resources.
- Conduct intake interviews to determine program eligibility from walk-ins and referrals. Maintain waiting lists and make referrals to other organizations as needed. Complete required paperwork, communicating program boundaries, rules, services, and limitations.
- Conduct outreach to external partners and organizations, providing education about available programs and services. Collect and organize community donations.
- Stay updated on community resources available to clients.
- Assist clients with transportation needs for housing, shopping, etc., utilizing Lancaster Transit and other options when possible.
- Establish and maintain client files, ensuring timely and accurate documentation, including case notes, referrals, and statistics.
- Monitor direct client assistance to conform to budget and keep accurate mileage documentation.
- Perform other duties as assigned.
Requirements for the ADAMH Case Manager
- Associate's Degree in Social Work or related field; Bachelor's preferred.
- 2-4 years of case management and/or crisis intervention experience.
- Experience with homeless or other special needs populations preferred.
- Flexibility to manage schedule including weekends and evenings.
- Valid Ohio driver's license, access to reliable insured transportation, and ability to meet insurance standards.
