Qualifications

Case Manager - ADAMH (Alcohol, Drugs & Mental Health)

Full Time Days

Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, providing meals and shelter to men, women, and Veterans with dignity and compassion.

What will I do as the ADAMH Case Manager with Lutheran Social Services?
  1. Work with clients regularly to develop and maintain personalized plans of action, assessing their strengths and needs, setting goals, and assisting with budgeting, job and life skills development, housing stability, and bill management.
  2. Make appropriate referrals and linkages to community resources, supporting clients in securing stable housing and income, and act as a liaison with outside resources.
  3. Conduct intake interviews to determine program eligibility, maintain waiting lists, and complete necessary paperwork while communicating program boundaries and services.
  4. Engage in outreach to community partners, providing education about available programs and collecting donations from the community.
  5. Stay informed about community resources to better assist clients.
  6. Assist clients with transportation needs for housing, shopping, and other activities, utilizing Lancaster Transit and other options when available.
  7. Maintain accurate and timely documentation, including case notes, referrals, and statistics, ensuring compliance with program requirements.
  8. Monitor direct client assistance to stay within budget and document mileage accurately.
  9. Perform other duties as assigned.
Requirements for the ADAMH Case Manager
  1. Associates Degree in Social Work or related field; Bachelors preferred.
  2. 2-4 years of case management and/or crisis intervention experience.
  3. Experience with homeless or other special needs populations is preferred.
  4. Flexibility to manage schedule including weekends and evenings.
  5. Valid Ohio driver’s license, access to reliable insured transportation, and ability to meet insurance standards.
