Job Title: Abstinence Case Manager

Program: In Homes Now

Location: 357 West 126th Street, New York, NY, USA

Salary Range: $47,000 - $55,000

Program Description:

In Homes Now is a Scatter Site Housing Program providing housing to clients in recovery, leaving the shelter system, with histories of homelessness, substance use, and mental health challenges. The program currently houses 85 clients across Brooklyn, The Bronx, and Manhattan. Our multidisciplinary team includes case managers, peer counselors, psychiatric care providers, and occupational therapy interns dedicated to ensuring housing stability and improving client wellbeing.

Overall Responsibilities:

This is a full-time position at PRI. The case manager will manage a caseload of 28-30 clients, meeting with each client at least twice a month. The role requires active fieldwork.

Essential Duties and Responsibilities:
  • Conduct monthly home visits to clients' apartments.
  • Conduct monthly office visits.
  • Perform annual housing inspections.
  • Counsel clients on issues such as money management, sobriety, employment, and other personal goals.
  • Facilitate caseload groups.
  • Maintain accurate and current clinical documentation and files.
  • Complete initial and annual assessments.
  • Provide referrals to community providers to ensure clients are connected to local services.
Qualifications:
  • Bachelor's degree required.
  • Must be bilingual in Spanish.
  • Minimum of two years of experience in the human services field.
  • Experience working with homeless individuals and/or those affected by substance abuse is preferred.
  • Excellent verbal and written communication skills.
