Qualifications

• High school diploma or GED required.
• 1–3 years of relevant experience in case management.
• Knowledge of client intake, assessment, service coordination, crisis intervention, and documentation.

Responsibilities

• Admit new clients by reviewing records and applications; conduct orientations; complete intake interviews; determine need for referrals and interventions.
• Assist clients by setting schedules and routines; coordinating services; arranging resources, including transportation.
• Monitor cases by verifying attendance; observe treatments; advocate for needed services and entitlements; obtain resources; intervene in crises.
• Maintain clients' records by reviewing case notes; logging events and progress.
• Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminate results and obstacles to the team and family.
• Prepare for discharge by reviewing discharge plans; coordinate discharge and post-discharge requirements; train family members; provide resources.
• Prepare reports by collecting, analyzing, and summarizing treatment data; compile statistics; complete grant and subsidy applications.

Position Summary

Dhaka Technologies Limited Company (DTLC) is hiring a Case Manager on behalf of a government client in Washington, DC. This is a full time, onsite position working 8 hours per day.

Duties and Responsibilities

• The Case Manager admits new clients by reviewing records and applications; conducting orientations; completing intake interviews; and determining the need for referrals and interventions.

Minimum Requirements

• High school diploma or GED
• Knowledge of case management practices, client intake and assessment, service coordination, crisis intervention, and client documentation 1 to 3 years of relevant experience
