Seeking aCase Managerto join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Recovery Health Center.

Full-Time Employed: 40 hours/week

Bilingual in Spanish is a plus (Additional Language Differential Pay Available)

Duties may include:
Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualizedservice plan are implemented
