Job summary

Adecco is seeking a Case Manager to coordinate patient care and facilitate smooth transitions from admission to discharge in a rehabilitation setting. You will work with patients, families, and an interdisciplinary team to develop plans and ensure timely services.

The role emphasizes assessment, care coordination, and ongoing education for patients and families, with opportunities for professional growth and a collaborative environment.

Qualifications

• Active license/certification appropriate to discipline.
• Ability to independently complete assessments within scope of practice.
• RN: Minimum Associate Degree in Nursing; BSN preferred.
• Other eligible healthcare professionals: Minimum Bachelor's degree; graduate degree preferred.
• Eligible for CCM® or ACM™ certification upon hire or within 2 years.
• 2+ years of rehabilitation experience preferred.

Responsibilities

• Coordinate patient care with an interdisciplinary team.
• Develop and monitor individualized treatment and discharge plans.
• Meet with patients and families to establish goals and communication plans.
• Facilitate weekly team conferences and family meetings.
• Coordinate referrals and post-discharge services.
• Monitor patient progress, utilization, and payor requirements.
• Support utilization review and identify opportunities to improve outcomes.
• Help prevent discharge delays by coordinating resources and services.
• Ensure accurate and timely case management and discharge documentation.
• Educate patients and families throughout the rehabilitation process.
• Participate in acute-care transfer reviews and identify trends.
• Stay current on applicable regulations, standards, policies, and payor requirements.

• Ability to independently complete assessments within your scope of practice
• RN candidates: Minimum Associate Degree in Nursing; BSN preferred
• Other eligible healthcare professionals: Minimum Bachelor's degree; graduate degree preferred
• Eligible for CCM® or ACM™ certification upon hire or able to obtain certification within 2 years
