As a Case Manager, you’ll serve as a critical connection between patients, families, providers, payors, and the interdisciplinary care team. You’ll help ensure patients receive the right care at the right time while supporting a smooth transition from admission through discharge.
What You’ll Do
- Coordinate patient care and collaborate with an interdisciplinary team
- Develop and monitor individualized treatment and discharge plans
- Meet with patients and families to establish goals and communication plans
- Facilitate weekly team conferences and family meetings
- Coordinate referrals and post-discharge services
- Monitor patient progress, quality, utilization, and payor requirements
- Support utilization review and identify opportunities to improve outcomes
- Help prevent discharge delays by coordinating resources and services
- Ensure accurate and timely case management and discharge documentation
- Educate patients and families throughout the rehabilitation process
- Participate in acute-care transfer reviews and help identify trends
- Stay current on applicable regulations, standards, policies, and payor requirements
What You Bring
- Active license/certification appropriate to your discipline (RN, SW, OT, PT, ST, Rehabilitation Counseling, etc.)
- Ability to independently complete assessments within your scope of practice
- RN candidates: Minimum Associate Degree in Nursing; BSN preferred
- Other eligible healthcare professionals: Minimum Bachelor's degree; graduate degree preferred
- Eligible for CCM® or ACM™ certification upon hire or able to obtain certification within 2 years
- 2+ years of rehabilitation experience preferred
