Job summary

The Adecco Group is seeking a compassionate Case Manager to join an inpatient rehabilitation team, guiding patients through recovery with care coordination and advocacy. You’ll work with an interdisciplinary team from admission to discharge, coordinating plans, education, and community resources to support optimal outcomes.

Ideal candidates hold an active healthcare license (RN, Social Work, PT, OT, ST, Rehabilitation Counseling, or equivalent) with nursing degrees required for RNs and a BSN

Qualifications

• Active license in nursing or rehabilitation field (RN, Social Work, PT, OT, ST, Rehabilitation Counseling, or other qualifying license).
• Associate Degree in Nursing required for RNs; BSN preferred for other eligible disciplines.
• Bachelor's degree required for other eligible disciplines.
• Rehabilitation experience preferred.
• Ability to obtain CCM or ACM certification within two years of hire.

Responsibilities

• Coordinate patient care throughout the rehabilitation process.
• Develop individualized care plans with physicians, nurses, therapists, and families.
• Lead interdisciplinary team meetings and treatment discussions.
• Manage discharge planning and connect patients with appropriate community resources.
• Educate patients and families on recovery goals and expectations.
• Monitor patient progress, insurance requirements, and care outcomes.
• Maintain accurate documentation and ensure regulatory compliance.
• Support utilization review and quality improvement initiatives.
