Job summary

A community support organization in Village of Shoreham is seeking a Case Manager responsible for providing case management services and advocating for clients. Ideal candidates must possess a Bachelor's degree in a related field and have at least 2 years of experience in working with individuals with substance abuse or mental health issues. The role includes individual counseling, documentation, and assisting clients with housing. A generous benefits package, including medical insurance and paid time off, is offered.

Responsibilities

• Provide case management services including service planning and medication monitoring.
• Deliver individual counseling and support services while handling documentation.
• Assist with housing services, including maintaining vouchers and resolving maintenance issues.
• Conduct unit visits weekly and inspections monthly.
• Participate in landlord and lease issue resolution.
• Ensure compliance with funding requirements and agency standards.
• Attend weekly supervision and staff meetings.
• Perform crisis intervention as needed.

Description

Concern Housing is seeking a Case Manager to be responsible for providing case management services. The case manager is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access. The case manager is also an advocate for their clients.

Days/Hours

Monday - Friday; 4pm - 12am

Duties and Responsibilities

• Provide case management services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, programs, vocational services and opportunities, community integration, entitlements etc.)
• Provide individual counseling and support services; responsible for all documentation.
• Housing services including assistance with maintaining their voucher, education assistance with the life skills needed to maintain an apartment and resolving maintenance issues.
• Provide unit visits weekly and unit inspections monthly.
• Participate in resolution of landlord and lease issues and coordination with public housing authority recertification.
• Complete all necessary documentation to ensure compliance with funding requirements and agency quality assurance standards.
• Attendance at weekly supervision and staff meetings.
• Crisis intervention.
• Remain current with all Agency required compliance documents and trainings.

Requirements

Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working with persons with histories of substance abuse and/or mental illness. Previous work with Veterans preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
