Job summary

Concern Housing seeks a Case Manager to provide direct case management services to Veterans at Liberty Village in Amityville, NY. You will advocate for clients and connect them with community resources, services, and supports.

Responsibilities include service planning, counseling, documentation, and housing assistance, such as maintaining VASH vouchers and addressing maintenance issues. You will work Monday–Friday, 9am–5pm, with a strong emphasis on compliance and coordination.

Qualifications

• Bachelor's degree in Psychology, Rehabilitation, Social Work, or a related mental-health field.
• Minimum 2 years of experience working with individuals with substance abuse or mental illness.
• Veterans experience is preferred.

Responsibilities

• Provide case management and service planning for Veterans at Liberty Village.
• Offer individual counseling and document all activities.
• Assist with housing services, including maintaining VASH vouchers and life-skills education.
• Conduct unit visits and coordinate with housing authorities.
• Ensure compliance with funding requirements and QA standards.
• Attend supervision and staff meetings and provide crisis intervention.

Overview

Concern Housing is seeking a Case Manager to provide case management services directly to Veterans at Liberty Village. The case manager acts as a resource for clients, facilitating access to community resources, services, and support, serving as an advocate.

Days / Hours

Monday – Friday; 9 am – 5 pm.

Duties & Responsibilities

• Provide case management services (service planning, medication monitoring, linkages and/or referrals to clinical services, programs, vocational services, community integration, entitlements, etc.).
• Provide individual counseling and support services; responsible for all documentation.
• Provide housing services, including assistance with maintaining a VASH voucher, education assistance with life‑skills needed to maintain an apartment, and resolution of maintenance issues.
• Conduct unit visits weekly and unit inspections monthly.
• Participate in resolution of landlord and lease issues and coordinate with the public housing authority recertification.
• Complete all necessary documentation to ensure compliance with funding requirements and agency quality assurance standards.
• Attend weekly supervision and staff meetings.
• Provide crisis intervention.
• Remain current with all agency‑required compliance documents and trainings.

Requirements

Bachelor’s degree in Psychology, Rehabilitation, Social Work, or another related mental‑health discipline, plus 2 years of experience working with individuals with histories of substance abuse and/or mental illness. Previous work with Veterans is preferred. The candidate must have strong verbal and written communication skills and the ability to make sound judgments regarding client care.

Benefits

Salary: $50,000 per annum.
