Job summary

North American Family Institute (NAFI) is seeking a full-time Case Manager to deliver Parenting with Love and Limits (PLL) to adolescent youth and families in Polk, Highlands, and Hardee Counties, Florida.

Requirements include a Bachelor's Degree in a related human service field, experience with at-risk youth, and a valid Florida driver's license with reliable transportation; evenings and travel to multiple sites may be needed, plus knowledge of electronic health records.

Qualifications

• Bachelor’s Degree from an accredited college in a related human service field.
• Experience working with at-risk adolescents.
• Valid Florida driver's license with clean driving record, reliable transportation.
• Willingness to drive to multiple sites, including transporting clients as needed.
• Computer experience and familiarity with electronic health records.

Responsibilities

• Deliver PLL to adolescent youth and families across Polk, Highlands, and Hardee Counties, Florida.
• Coordinate services and transportation to multiple sites as needed.
• Support and monitor progress of youths and families in the PLL program.

North American Family Institute (NAFI) is accepting resumes for a full-time Case Manager for the delivery of Parenting with Love and Limits (PLL) to adolescent youth and families living in Polk, Highlands, and Hardee Counties, Florida. PLL is an evidence-based community program. NAFI works with youth and families involved with the Florida Department of Juvenile Justice.

All applicants will have a Bachelor’s Degree from an accredited college in a related human service field, and experience working with at-risk adolescents. This position requires a valid Florida driver’s license, a clean driving record, reliable transportation, and the willingness to drive to multiple sites, including transporting clients, as needed. This position also requires the ability to work a flexible schedule, including evening hours, as needed. Computer experience and familiarity with electronic health records is required.

Compensation for this position is $18.54 per hour.
