Advantage Care Group is seeking a Case Manager in Pennsylvania to develop therapeutic relationships with students and families, ensuring all mandated counseling services are delivered as per each student’s IEP. The role includes coordinating services, communicating with guardians, and reporting on plan development and maintenance.
The position involves intake, record management, scheduling, discharge planning support, and accompanying clients to court, medical appointments, and school meetings.
Position Summary
The Case Manager will develop and nurture therapeutic relationships with students and their families; assure all mandated counseling services as described by a student’s Individualized Education Plan are received; deliver program‑driven related services; maintain appropriate communication with parent/legal guardians; evaluate and report information appropriate to individual plan development and maintenance; participate in linkages between the residence and outside social services; coordinate, secure, evaluate, and report applicable social service information; develop, support, and maintain continuity of services between the residence and outside social services.
Working environment is generally favorable; there is the potential for adverse and/or hazardous events related to working with the youth. Advantage Care Group is located in a private community setting.
Position often requires lifting up to 10 lbs. and intermittent sitting, standing, bending and walking; requires good visual and auditory acuity for monitoring youth behavior. In an emergency, the employee must be able to physically respond and assist in evacuation or rescue, bend, lift, run or walk swiftly in order to intervene in a combative situation(s) to protect fellow employees or other youth from the threat of harm or actual harm at the hands of youth. Must have the ability to administer First Aid and CPR.
Thorough understanding of age, cognition, and developmental stages of school‑age children. Effective communication skills and ability to quickly form trusting relationships with child/adolescent population. Attend and participate in all in‑service training programs.

Case Managers will work an 8‑hour shift, preferably 8‑am until 5:00 pm daily, however due to the nature of this position, there may be unforeseen occasions where the hours will need to be flexible and may exceed the 8‑hour shift providing continuous client supervision. Employees may be required in an emergency to be retained for a brief period after their regular shift.
