Job summary

Thrive Behavioral Health in Rhode Island seeks a Case Manager-Residential to provide direct services and coordinate comprehensive care for clients and their families. You will link clients to mental health, medical, and social services while advocating for recovery and independence.

Responsibilities include coordinating care, monitoring psychiatric status and medications, developing recovery plans, and assisting with housing and income management.

Qualifications

• Bachelor's degree in human services or equivalent.
• Two years’ experience in human services with clients who have disabilities preferred.
• Experience or training in related state and federal assistance programs preferred.
• Basic computer literacy required.

Responsibilities

• Provide assessment, referral, education, treatment, and follow-up services for mental health, medical, and substance use needs.
• Collaborate with providers, agencies, and the client’s network of care.
• Provide referral, advocacy, and education on chemical dependency or mental health issues.
• Monitor psychiatric signs, medications, and document findings; report variances to clinical supervision.
• Teach clients symptom management and medications as prescribed.
• Help clients develop and strengthen their support networks.
• Assist with living arrangements and income maintenance for safe housing.
• Manage and educate on client rep payee accounts.
• Develop and implement client-centered recovery plans and document services per Thrive standards.
• Complete billing forms and maintain client records in compliance with regulations.
• Schedule client hours to meet billing targets.
• Participate in clinical team meetings for coordinated care.
• Provide crisis intervention and accompany clients to appointments.
• Coordinate and oversee clients’ physical healthcare needs.
• ACT Team members may work nights, weekends, and holidays per regulations.
• Review email daily.

Education

Bachelor's Degree in the Human Service field

CASE MANAGER-RESIDENTIAL

FULL TIME - NON EXEMPT

General Summary: Provide direct service, coordination of care, and linkage to services andadvocacy, as needed to individual client and/or family for the purpose of assisting client with their overall life management and recovery.

Essential Responsibilities

• Provide assessment, referral, education, treatment, and follow-up services to clients on issues concerning mental health, medical, substance abuse, vocational, and living skills.
• Collaborate with other providers, agencies, and individuals in the client's network of care.
• Provide referral, advocacy and education to the community regarding issues of chemical dependency or mental health.
• Monitor signs and symptoms of psychiatric functioning; monitor medication regimen and document findings. report any variance to appropriate clinical supervisory staff.
• Teach clients effective techniques for symptom management and medication as prescribed.
• Assist clients in developing and strengthening their support network to accomplish goals and objectives.
• Provide assistance with living arrangements and income maintenance to insure safe and affordable housing.
• Provide assistance and education in the management of client rep payee accounts.
• Develop and implement treatment person centered recovery plans and document services consistent with Thrive standards.
• Produce, complete, and maintain billing forms, logs, data collection forms, clinical documentation and client records on time and in accordance with Thrive standards and state and federal regulations.
• Schedule and maintain client hours sufficient to achieve individual / program billing targets.
• Participate in clinical team meetings to ensure smooth coordination of client services and treatment.
• Provide crisis intervention.
• Accompany client to community appointments, provide advocacy as needed.
• Assist in coordination and oversight of client's physical healthcare and needs.
• Assertive Community Treatment (ACT) Team members are required to work nights, weekends, and holidays, per BHDDH regulations.
• Review e-mail account daily for new messages.

Additional Responsibilities

• Attend all agency/department required training and meetings within the prescribed period.
• Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.

Qualifications

Education, Experience and Competencies:

• Bachelor's Degree in the Human Service field or an equivalent combination of education and experience required.
• Minimum (2) year experience in Human Service field working with clients with disabilities preferred.
• Experience or training in related state and federal assistance programs also preferred.
• Basic computer literacy skills are necessary.

Certifications, Licenses Requirements:

• Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
• Case Management /Community Support Professional Certification or working toward preferred.
