Job summary

Chief Seattle Club is seeking a Full-Time Case Manager in Seattle, WA to work on-site in permanent supportive housing with residents and a team of Support Assistants. You will perform screenings, coordinate referrals, and support residents in achieving housing, health, and life goals within a culturally adaptive wrap-around service model.

The role emphasizes strong crisis intervention, documentation accuracy, and collaboration with program leadership.

Qualifications

  • Bachelor’s degree in social work or related field; master’s preferred.
  • Minimum 2 years case management experience with vulnerable populations.
  • Basic knowledge of computers and Microsoft Office 365.
  • Knowledge of housing policies, rights, and applicable laws.

Responsibilities

  • Preliminary screening with residents to assess needs (housing, employment, education, health, safety).
  • Provide referrals to outside agencies to remove barriers to basic needs.
  • Assist residents in creating goals or action plans and follow up regularly.
  • Collaborate with CSC staff to deliver wrap-around support services.
  • Maintain documentation using HMIS/Clarity or Agency systems.
  • Create and report monthly data/statistics for leadership and board reports.

Job Summary:

Chief Seattle Club provides permanent, affordable, and supportive housing to residents (Native American/Alaska Native) at our facilities. Along with housing comes wrap-around support services to help residents live healthy and sustainable lives.

Under the direction of the Program Manager, the Case Manager works in permanent supportive housingwith the Support Assistantsand residents on supporting life at our housing sites, which includes culturally adaptive wrap-around services.

Goals and objectives are established and evaluated by the Program Manager, Residential Services Director, and Supportive Housing Director. This position requires on-site work.

Essential Job Functions:
  • Preliminary screening with current residents of the community to assess needs (housing, employment, education, domestic violence advocacy, mental & spiritual health, substance abuse treatment, health, parenting referrals).
  • Provide referrals to outside agencies or organizations to resolve barriers to basic needs.
  • Assist members in creating goals or an action plan and continually follow up with residents.
  • Work with other CSC staff to deliver support services.
  • Assist with residents and the Advocate team with move-in/move-out inspections as needed.
  • Coordinate and network with fellow case managers to develop any necessary new resources.
  • Maintain and complete necessary documentation through client tracking system such as HMIS/Clarity or Agency.
  • Create and report monthly data/statistics to Program Manager for Board reports.
  • Participate in staff/team meetings.
  • Participate in residential activities and events.
  • Support Advocates with any resident needs.
  • Support and maintain a therapeutic and healing work environment.
  • Create and maintain resident files in secure areas such as file cabinets and/or a Cloud platform.
  • Assist the Program Manager with any move-ins/move-outs.
  • Actively participate in safety-related activities, including attending safety trainings, adhering to safety protocols, and serving as a member of the organization’s safety committee to ensure a safe environment for staff and clients.
  • Perform other duties as assigned.
Knowledge, Skills and Abilities Required
  • Bachelor’s degree in social work, Human Services, Psychology, or a related field (master’s degree preferred).
  • Minimum 2 years case management experience working with vulnerable populations and/or working in a non-profit social service setting.
  • Basic knowledge of computers and Microsoft Office 365.
  • Knowledge of current policies, laws and rights around housing.
  • High degree of discretion and excellent judgment.
  • Ability to handle complaints/conflicts in a calm and professional manner.
  • Strong crisis intervention skills with the ability to prioritize and handle multiple tasks under stressful conditions.
  • Must have a strong understanding of historical and intergenerational trauma among Indigenous communities.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Excellent verbal and written communication skills.
Licenses/Certifications/Training:
  • CPR/First Aid

Physical Requirements:
  • Ability to sit, stand, and walk for extended periods while working with clients and staff.
  • Frequent use of a computer, phone, and other standard office equipment.
  • May require occasional lifting (up to 20 lbs.).
  • This position typically operates in a professional office setting but may require visits to other CSC facilities, partner agencies, and community organizations.
  • Collaboration with other team members, external agencies, and landlords to advocate for and support clients.
  • Most positions at CSC are exposed to members of the community who have experienced or are experiencing trauma in various forms including but not limited to domestic violence, sexual violence, homelessness, unemployment, financial hardship, etc. As a result, staff is at risk of secondary trauma. Employees are encouraged to seek external support and maintain self-care when working indirectly or directly with clients. Mental health care referrals for employees are available.
