Mercy Rehabilitation Hospital; Northwest Arkansas is seeking a Case Manager to guide patients and families through the inpatient rehabilitation journey, ensuring seamless communication, effective care planning, and smooth transitions.

Join a collaborative, patient‑centered rehab team with opportunities for professional development as you coordinate care, document IPoCs per CMS, and lead weekly conferences to support optimal patient outcomes.

Qualifications

• Active licensure as an RN, Social Worker (LSW), Respiratory Therapist, PT, OT, or SLP.
• 2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred).
• Strong communication, organization, and collaboration skills.
• Proficiency in Microsoft Office.
• Case Management or Rehab Nursing certification (CCM, ACM, RN‑BC, ARN); preferred.

Responsibilities

• Lead interdisciplinary care coordination and discharge planning.
• Identify barriers to progress and help develop solutions with the care team.
• Document care activities and complete the IPoC per CMS guidelines.
• Facilitate weekly team conferences and family meetings.
• Communicate patient progress and discharge plans to caregivers.
• Review CMGs and collaborate with HIM on comorbidities.

How you’ll contribute:

• Facilitate weekly team conferences and family meetings
• Communicate patient progress and discharge plans to caregivers
• Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
• Support additional departmental needs as assigned
