Job summary

Point Healthcare Services in Renton, WA is seeking a dedicated Case Manager to join our team. You will coordinate high-quality, person-centered care, assess patient needs, and advocate for patients and families to connect them with community resources.

You will work with physicians, social workers, and care teams to develop care plans, document progress, and support transitions of care, ensuring continuity and quality across services.

Qualifications

• Bachelor's degree in Social Work, Nursing, Healthcare Administration, Medical Assistant, or a related field.
• 1-3 years of experience in case management, care coordination, or a similar healthcare role.
• Knowledge of community resources, healthcare systems, and social determinants of health.
• Strong communication, organizational, and problem-solving skills.
• Proficiency with electronic health records (EHR) and case management software.
• CCM certification is a plus.
• Empathetic, patient-focused demeanor with ability to work with diverse populations.

Responsibilities

• Assess patient needs and develop individualized care plans to support health and wellness goals.
• Coordinate and monitor healthcare services, referrals, and community resources for assigned caseload.
• Serve as a primary point of contact and advocate for patients, families, and caregivers.
• Collaborate with multidisciplinary care teams including physicians, social workers, and specialists.
• Maintain accurate and up-to-date case documentation in compliance with regulatory standards.
• Conduct follow-up assessments to evaluate patient progress and adjust care plans as needed.
• Facilitate transitions of care and discharge planning to ensure continuity of services.
