PURPOSE OF POSITION

The Case Manager provides case management, and recovery-oriented services to individuals experiencing persistent mental illness or emotional and behavioral disorders. Working under general supervision, this position supports individuals in achieving stability, independence, and improved quality of life by facilitating access to appropriate treatment and community resources. The role ensures services align with agency standards and contribute to the overall well-being of Grant County residents.

DISTINGUISHING CHARACTERISTICS

This position is part of the Agency’s clinical service team and focuses on providing ongoing case management and rehabilitative support to individuals with significant mental health needs. Unlike therapy-focused positions, the Case Manager emphasizes coordination of care, resource linkage, skill building, and recovery support in community and office settings. The role requires independent judgment within established policies, regular field work, and close collaboration with clinicians, medical providers, social service partners, and other community agencies.

• Provides individual and group treatment services and case management interventions to support client stability and promote recovery.


• Assures that all documentation and records meet Washington Administrative Code (WAC), Revised Codes of Washington (RCW), regional and state contracts, and all other applicable Agency standards to maintain regulatory compliance.


• Meets established competency levels for service coding, billing requirements, and other direct service standards to support accurate and timely reimbursement.


• Prioritizes treatment needs by applying life-domain data to develop age-, culture-, financial-, and disability-specific goals and measurable treatment objectives that guide care.


• Monitors client risk related to harm to self, others, property, or grave disability and refers to appropriate services to ensure safety and stabilization.


• Advocates on behalf of individuals to coordinate internal and external services that align with treatment goals and improve outcomes.


• Provides education to respective clients and encourages the client while documenting the clients' understanding of medication needs. Advises client of illness warning signs to promote adherence and self-management.


• Reports and consults with prescribers, as needed, to support safe medication practices.


• Utilizes crisis intervention techniques to stabilize individuals and the community during periods of acute need.


• Conducts discharge planning and treatment closure to support smooth transitions and continuity of care.


• Collaborates with staff to follow quality improvement and utilization management standards that enhance service effectiveness and compliance.


• Documents services in client records within established agency timelines to maintain accurate, current, and complete clinical information.


• Delivers services that respect individual rights and reflect cultural and linguistic appropriateness to ensure equitable, person-centered care.


• Participates in required training and meetings, whether on-site or off-site, to maintain professional competency and program alignment.


• Performs other duties as assigned.

Required Education and Experience

• Bachelor's degree in psychology, social work or closely related field


• Experience conducting clinical assessments including crisis evaluations, risk assessments, and diagnostic assessments.

Preferred Education and Experience

• Previous case management related experience


• Experience working in a bilingual or bicultural environment.


• Experience working within community-based mental health or county/regional behavioral health systems.

Professional Licenses/Certifications

• Washington State Registered Agency Affiliated Counselor Credential

Other Requirements

• Valid Washington State Driver's License with proof of automobile insurance and acceptable driving abstract.

WORKING CONDITIONS AND ENVIRONMENT

• May encounter individuals experiencing emotional distress, hostility, or aggressive behavior.


• May occasionally work in community settings such as provider offices, schools, jails, residential facilities, client homes, or meeting sites.


• May encounter varied environmental conditions when working off-site.


• May experience interruptions or moderate noise levels in shared work environments.


• Duties may require travel within the region or state, including driving in varying road and weather conditions and attending trainings or meetings as needed.
