Qualifications

• High school diploma or GED required.
• Must be BHT with two years of related behavioral health work experience, or a related bachelor’s degree (preferred).
• Must be 21 years or older; valid Arizona Driver's License with a 39‑month driving record; reliable transportation; CPR and First Aid training.

Responsibilities

• Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.
• Collaborating with the family and team members to create support plans that capture strengths, needs, and culture of the family.
• Facilitating Child and Family Team meetings; establishing an agenda, keeping to timeframes, and building consensus for children and families.
• Assisting young people and families in securing needed community resources and linkages to other services.
• Interacting professionally with stakeholders (DCS, DDD, juvenile probation, Direct Service Providers, etc.).
• Performing other duties related to the position.

Case Manager (Full‑time)

Child & Family Support Services (CFSS) in Yavapai County is accepting applications for a Case Manager. The role is ideal for a determined, experienced behavioral health technician, social worker, or behavioral health professional seeking a rewarding career working with young adults and their families. This is not a supervisory position. The majority of time is spent in homes and the community rather than an office setting.

Case Manager Job Responsibilities

• Providing ongoing case management; coordinating behavioral health services, medical care, education, placement, finances, and socialization services to children and families with intense/complex needs.
• Collaborating with the family and team members to create support plans that accurately capture the strengths, needs, and culture of the family.
• Facilitating Child and Family Team meetings; establishing an agenda, setting and maintaining a strength‑based meeting environment, staying within timeframes, giving all participants the opportunity to contribute, holding people accountable for commitments, and assisting a team in developing consensus for children and families.
• Assisting young people and families in securing needed community resources and linkages to other services. Helping families to identify natural and informal supports.
• Service delivery can be inclusive of skills training and family support.
• Interacting professionally with other stakeholders: Division of Child Safety (DCS), Division of Developmental Disabilities (DDD), juvenile probation, Direct Service Providers (DSP), etc.
• Performing other duties related to the position.

Schedule and Travel Requirements

This position requires working afternoons, evenings, and weekends. Adaptability in scheduling, driving throughout the county, and working within the homes and communities of the families we support is required.

Education and Experience Needed

• High school diploma or GED.
• Must be BHT with two (2) years of related behavioral health work experience, or a related bachelor’s degree (preferred but not required).

Case Manager Requirements

• Must be 21 years or older.
• Valid Arizona Driver's License with an acceptable 39‑month driving record.
• Reliable transportation, including proof of insurance.
• Submit a negative drug test.
• Obtain an Arizona DPS Level One Fingerprint Clearance Card.
• Complete Adult, Child, and Infant CPR and First Aid training.
