Job summary

Rehab Without Walls, an affiliate of BrightSpring Health Services, is seeking a Case Manager in Portland, OR to coordinate comprehensive neurorehabilitation services in home and community settings.

You will assess medical, social, and psychological needs; develop individualized care plans; and advocate for patients as they access therapy, medical care, and support services.

Qualifications

• Bachelor's or Master's degree in a related field preferred.
• Clinical case management experience, preferably in a rehabilitation or healthcare setting.
• Knowledge of brain injuries and rehabilitation preferred.

Responsibilities

• Advocate for patients' rights and ensure their needs are met throughout rehabilitation.
• Conduct comprehensive assessments of patients' medical, social, and psychological needs.
• Collaborate with the treatment team to develop individualized care plans.
• Coordinate and monitor the delivery of services, including therapy, medical care, and support services.
• Provide education and support to patients and their families.
