Victory Starts Now INC in Pasadena, CA seeks a Case Manager to oversee the full care cycle for members, including planning, implementing, monitoring and evaluating actions to meet health and social needs.

The role requires strong organization, empathy, and the ability to coordinate services, advocate for low income or homeless clients, and maintain professional boundaries. A valid driver’s license and reliable transportation are preferred, with Spanish/English bilingual skills highly desirable.

  • Bilingual (Spanish/English) highly desirable.

Responsibilities

  • Inspect and check members’ identifications.
  • Obtain and assist in the facilitation of members’ birth certificates, identification cards, medical and Social Security benefits cards, and transportation cards.
  • Conduct members’ needs assessments, including basic physical and social needs.
  • Develop plans to assist members achieve their life goals and objectives.
  • Run meetings with members every Monday and Friday to obtain updates on members’ personal developments and achievements and events in the house.
  • Properly document members’ files.
  • Make appropriate referrals and follows up to assure delivery of service to each member.
  • Present summary and status of cases on weekly Case Review Meetings with supervisors and/or company management.
  • Attend and satisfactorily complete periodic trainings and conferences and impart information with company staff as needed.
  • Network with other social service agencies.
  • Confer with team on a regular basis to assess members’ participation, conduct audits and revise service plan as necessary.
  • Maintain log to track members’ appointments, including but not limited to medical and court appointments.
  • Coordinate, transport and accompany members to their appointments.
  • Comply with the company’s expense reimbursement policy, including submission of timely requests and supporting documentation.
  • Assist with other related duties as required.
