Position Description

The RN Case Manager works under the guidance of the Director of Case Management. The RN Case Manager collaborates with other healthcare team members to coordinate and facilitate the effective delivery of quality healthcare services to Palmdale Regional Medical C enter patients within the limits of available resources. As a member of the interdisciplinary team, the RN Case Manager focuses on the achievement of positive outcomes for the patient, the provider, the facility and the payer. The RN Case Manager is knowledgeable about and responsible for utilization review, discharge planning and case management of Palmdale Regional Medical Center‘s patients regardless of the patient’s condition, religion, ethnic background or payer.
