We are seeking a dedicated and detail-oriented Case Manager to oversee all active and new cases within our
agency. The ideal candidate will ensure compliance with regulatory requirements, manage Direct Support
Professionals (DSPs), and maintain strong relationships with participants, guardians, and support coordinators. This
position requires excellent organizational skills, attention to detail, and the ability to multitask effectively.
Key Responsibilities:
Case Management:
• Oversee all active cases, ensuring compliance with monthly CRO, OIG, Medical Debarment, and Ineligible
Provider reports.
• Monitor DSP performance to ensure authorized hours for participants, maintaining clock-in and clock-out
records in HHA with an adjustment rate under 15%.
• Ensure DSPs submit service notes for all scheduled visits in compliance with DDD regulations.
• Conduct monthly check-ins with participants and/or guardians to verify that services align with Individualized
Service Plan (ISP) goals and outcomes.
New Case Onboarding:
• Facilitate introductory calls with guardians and/or participants to gather all pertinent information for effective
DSP staffing.
• Manage the entire case lifecycle from referral receipt to activation, including conducting quality checks once
cases are active.
Staffing and Training:
• Hire and onboard DSPs to ensure quick turnaround times for active referrals.
• Ensure all mandated checks and training for DSPs are completed.
Relationship Building:
• Conduct outreach calls and emails to support coordinators to establish and nurture relationships with the
intention of obtaining referrals for agency growth.
Qualifications:
• Bachelor’s degree in social work, human services, or a related field preferred or 3 years’ experience with
DDD Policy and Procedures.
• Proven experience in case management, particularly within DDD or similar agencies.
• Strong attention to detail and organizational skills.
• Excellent problem-solving abilities and multitasking capability.Plus
• Effective communication and writing skills.
• Personable and able to build rapport with diverse individuals.
• Bilingual in Spanish is a Plus
