Job summary

TALENT Software Services in Morristown, NJ, seeks an experienced discharge planning coordinator to oversee transitions from acute care hospitalizations. You will coordinate patients through the hospitalization process and manage all discharge planning activities to ensure timely, safe transitions.

Qualifications

• 2+ years of acute care experience preferred.
• Experience in Case Management and Managed Care.
• In-Patient Case Management experience required.
• Hospital experience including discharge planning and rehab placement.
• Minimum 2 years in acute inpatient discharge planning.
• Experience with subacute patients required.

Duties

Schedule

• Schedule: 8:00 AM - 4:00 PM
• Rotating weekends: Every 4th or 5th weekend
• Holidays: One summer and one winter holiday
