Job summary

SOS Wellness, a growing behavioral health practice serving Southwest Ohio, seeks dedicated Case Managers (CPST / TBS) to provide integrated, dignified care. Qualifications

  • Active Ohio Licensure/Registration: relevant credentials preferred (e.g., LSW, LPC, CDCA, or qualified behavioral health provider registration in Ohio).
  • Strong communication and advocacy skills paired with warmth and client-centered approach.
  • Comfortable with diverse payer mixes, including Medicaid and commercial populations, with accessible, dignified care.

Responsibilities

  • Provide high-quality CPST and TBS to individuals and families.
  • Collaborate with therapists, nurse practitioners, and Medical Director to coordinate care plans.
  • Actively connect clients to housing, healthcare, employment, and educational resources.
  • Empower clients with independent living skills, coping strategies, and self-advocacy.
  • Maintain clear, timely documentation within the EHR reflecting regulatory excellence.
  • Work across Middletown, Dayton, or Norwood with hybrid remote/telehealth flexibility.

Key Responsibilities
  • Provide high-quality community psychiatric supportive treatment (CPST) and therapeutic behavioral services (TBS) to individuals and families.
  • Collaborate dynamically with therapists, psychiatric nurse practitioners, and the Medical Director to coordinate comprehensive care plans.
  • Actively connect clients to vital community resources, including housing, healthcare, employment, and educational support.
  • Empower clients by teaching independent living skills, coping strategies, and self-advocacy within their environment.
  • Maintain clear, timely, and clinically accurate documentation within our EHR system that reflects regulatory excellence.
  • Practice from or across any of our three Southwest Ohio locations (Middletown, Dayton, or Norwood), utilizing hybrid remote or telehealth flexibility as appropriate for client needs.
Qualifications
  • Active Ohio Licensure/Registration: Relevant credentials preferred (e.g., LSW, LPC, CDCA, or qualified behavioral health provider registration in Ohio).
  • Strong communication and advocacy skills paired with the kind of warmth that puts clients at ease immediately.
  • Genuine comfort working across diverse payer mixes, including both Medicaid and commercial populations, with a strong commitment to accessible, dignified care.
  • Ability to successfully pass a comprehensive background check and drug screening.

  • Comfortable with modern EHR systems and digital collaboration tools.
Bonus Points
  • Minimum of 2 years of dedicated case management or community mental health experience (Preferred).
  • Direct familiarity with Valant EHR.
  • A background or strong interest in addiction treatment or intensive outpatient programs (IOP).
